Our health hubs are growing nation wide. Working with community based non-profit organizations (CBOs), we and organize a local coalition to support the improved health of engaged Medicaid clients. The model centers around acquiring housing for these clients, and wrapping around non-medical and medical supports to ensure rapid and successful stabilization. Since starting this work in 2022, we’ve driven over $1 Million towards CBOs delivering services around these health related social needs.
